Search for Submitted Claims Page

You use the Provider Submitted Claim Inquiry page to search for a submitted claim in order to view it.

The Search for Submitted Claims page contains the following panels:

Billing Provider Number Fields

Field Description
Billing Provider # The number of the billing provider to whom the claim is paid.

 

Additional Information Fields

Field Description
Member ID The ID of the member for whom the claim was filed.
Claim Submission Begin Date

The date the claim was submitted. If only the Begin Date (and no End Date) is entered, the search will assume the End Date is the same as the Begin Date.

Note: Submitted claims are kept on record for only a client-defined number of business days after the date of submission.

Claim Submission End Date The date the claim submission ends, after which the claim is no longer on record.
Claim Service Period Begin Date The date the claim service period begins.
Claim Service Period End Date The date the claim service period ends.

 

Search Results Fields

Field Description

If more than one match is found, this table is displayed. To view a match, click the appropriate row. None of these fields can be edited.

Provider ID The number of the billing provider to whom the claim is paid.
Member Name The last and first name of the member for whom the claim was filed.
Member ID The ID of the member for whom the claim was filed.
Claim Service Period Begin Date The date the claim service period begins.
Total Charges The total amount charged on the claim.
Claim Type

The type of claim.

Examples: Institutional, Professional, Dental

Claim Submission Date The date the claim was submitted.
Last Updated By The user ID of the last person to update the claim.
