Create Template - Enter Template Name Page

You use the Create Template page to create a new claim template to use as a basis for adding a new claim at a later date. The type of the claim (professional, institutional, or dental) is inserted in the title of the page and is based upon the sub-menu item selected from the Create Template menu.

Create Template Fields

Field	Description
Template Name	The name of the professional, institutional, or dental template you wish to create.
Long Description	Description of the type of template and reason for creating it.
Characters Remaining	Counter that indicates how many characters you have available when entering text. It decreases by one for each character entered and increases when a character is deleted.
